Please note, there is a £75 administration and settling-session fee for each child, payable by card or bank transfer using the details at the bottom of this form. Payment is required before a visit can be arranged and for your child to be placed on the waiting list. This fee will be refunded to you if you notify us within 7 days of your visit that you do not wish to take up a place.
If you are applying for more than one child, please complete a separate form for each child.
