Please note, there is a £75 administration and settling-session fee, payable by card using the details at the bottom of this form. Payment is required before a visit can be arranged and for your child to be placed on the waiting list. This fee will be refunded to you if you notify us within 7 days of your visit that you do not wish to take up a place.
If you are applying for more than one child at the same time, please complete a separate form for each child. However, you only need to pay the admin fee once.
